I. Apply online
for program approval.
The program must be approved for submission via the online grant application
form in order for the full grant application to be accepted.
Fill out the grant application form below and submit it online by March
15th for the May 1st deadline and by September
15th for the November
1st deadline. All submissions will be notified of their application
status 30 days before the application deadline. No hard copy grant
application packets will be accepted unless the program is approved
through the online application.

Filling out the application
form is easy by using the template form linked above.
A. Open the application form on your computer and type the information
in the boxes provided. When you click "submit", this will
come to the ELA Foundation directly. The complete application form
will appear on your screen ready to be printed once you click "submit".
B. Print the completed application form and retain it to be included
in your hard copy grant application packet.
Grant Pre Review:
The ELA Foundation offers the unique opportunity for grant applicants
to have a pre-review of their application by ELA Foundation staff.
Once your program is approved for application you may send the completed
grant application packet to the ELA Foundation a month in advance
of the deadline for a pre-review of your grant. The pre-review
deadline for May 1st is April 1st and for November
1st is October 1st. This will give the
ELA staff time to request any additional materials and will give
you time to complete the application before the deadline.
II. Complete the Grant application process
Once the program is approved, complete the summary and enclose it
with the financial information, attachments and printed grant application
form.
A. Summary
The Summary must be in 16-point type and double-spaced. Information
must include:
- Purpose of project
- Amount of grant requested
- Explanation of how this project will
change the face of disability in the community
- Population served
- Expected outcomes
- Program target dates
- Evaluation plan
B. Financial Information
- Program budget, including income
and expenses
- Funding plan for this program, including other funding
sources in the community
- Organizational budget, including income
and expenses for current fiscal year
- Audited financial statement
for most recent fiscal year
C. Attachments
- US 501(c)(3) Letter
- Printed materials, including Annual report,
organizational brochure, newspaper articles or other materials
- ADA
verification of compliance* - Include your answers to questions
below
- Emergency Preparedness plan** See web sites below for

- List of donors to the organization
- List of Board of Directors

VERIFICATION OF COMPLIANCE:
The ADA recommends four areas for becoming an ADA compliant organization:
Physical access, Programmatic access, Organizational access, and
Communication access. The following questions must be answered
as an attachment to
verify ADA compliance. These questions may be cut and pasted into

How does this organization market programs to people with disabilities
in the community? What alternative formats of communication
are used in printed programs and outreach materials?

III. Send your completed grant application to the ELA Foundation
by the deadline.
Grant deadlines are:
May 1st – 5 PM (Pacific Time)
November 1st – 5 PM (Pacific Time)
All proposals are due by 5 PM on the grant deadline date. No faxes
or hand deliveries will be accepted. To assure that your packet arrives
in time, send it via Federal Express, UPS, or guaranteed mail or
two weeks in advance by regular mail.
Occasionally the deadline falls on a weekend. Please plan accordingly
and get your packet to us before that date.
Grant recipients will receive notification of grant awards within
six weeks of the deadline.
